(he's fucking sick. the joys of loving and being loved in return, and also, merely thinking that he's not loved back. absolutely thrilling.
he hates this. big means that leaving his home is being very hard, and he's breaking out fevers, having difficulty breathing, and much worse than he has been in his life. that says something.)

p.s.: my little sister used to read stories about the whole coughing flowers situation. it's called hanahaki. something something, one-sided love, get that person to love you or die trying but literally? i don't remember much else. the younger japanese people might know better.
